Trastuzumab deruxtecan (Enhertu®) has been hailed as a revolutionary breakthrough in treating HER2-low metastatic breast cancer, an aggressive form of the disease. Despite its proven success in extending the lives of patients with complex prognoses, Spain’s healthcare system has decided not to fund this treatment, causing significant discontent among patients and specialists. However, the science behind this drug continues to offer hope for thousands of women around the world.

Breast cancer remains one of the leading causes of death among women globally. With advancements in medicine, new treatments are targeting the most resistant forms of the disease. One such treatment is trastuzumab deruxtecan (Enhertu®), a drug specially designed for patients with HER2-low metastatic breast cancer, a category difficult to treat due to its resistance to many conventional therapies.

This drug has demonstrated in clinical trials the ability to extend patients’ lives by more than six months, while also improving their quality of life. This is particularly significant for women who no longer respond to previous treatments. In European countries like Germany and France, Enhertu® has been approved for use and is funded by their healthcare systems.

However, in Spain, the situation is different. Despite approval by the European Medicines Agency, Spanish healthcare has refused to fund the drug due to high costs, leading to deep disappointment among patients and the medical community.

Despite this administrative decision, the future of trastuzumab deruxtecan remains promising. Oncologists assert that these types of drugs represent a new horizon in treating difficult cancers. “Precision medicine, based on targeted therapies like Enhertu®, is the path toward making metastatic breast cancer a manageable chronic illness rather than a death sentence,” explained one of the lead oncologists involved in the drug trials.

Meanwhile, breast cancer patient associations have raised their voices, arguing that the lives and well-being of thousands of women depend on the implementation of this drug in the public system. Many have shared their experiences, highlighting the improvement in their quality of life and renewed hope thanks to innovative therapies.

Impact on patients' lives

For many women, receiving a diagnosis of metastatic breast cancer is devastating, as it means the cancer has spread to other parts of the body, significantly reducing treatment options and prognosis. However, breakthroughs like trastuzumab deruxtecan have opened a new door toward controlling this disease. By specifically targeting HER2-low cells, the drug acts like a “Trojan Horse,” delivering a cytotoxic payload directly into cancer cells while minimizing damage to surrounding tissues.

Patients who have received the drug in clinical trials report more energy, less pain, and, most importantly, a significant reduction in tumors. “It’s a ray of light at a time when I thought there were no more options,” said one trial participant, whose cancer had stopped responding to other treatments.

A future with more hope

Despite current barriers, science is advancing rapidly. Research on metastatic breast cancer is constantly evolving, and drugs like trastuzumab deruxtecan represent a milestone in the fight against the disease. It’s likely that, over time, more similar treatments will emerge and become available to all patients, regardless of where they live or their economic situation.

The impact of these treatments is not only measured in additional months of life but in the quality of those months. For women facing a devastating diagnosis, every day counts. The hope is that with each medical advancement, life prospects improve, offering more time to spend with loved ones and, in many cases, more time to continue fighting.
